General Liability Claim Reporting Form Process
1. Complete the form and return to the Department of Student Housing along with any documentation of claim (pictures, emails, etc.). The form must be signed and dated by the resident.
2. The completed form, supporting documentation, and a copy of your housing contract will be submitted to the Office of Procurement Services at 8:00am on May 18. The files that you have submitted will then be distributed through the appropriate channels for review.
3. You may be contacted by the insurance company to provide further information and will determine if there is a claim.
